Job summary

Southwest Behavioral & Health Services seeks a dedicated Care Coordinator to support crisis stabilization, navigation of community resources, and coordination with the clinical team from our Prescott Valley Outpatient Clinic.

The role emphasizes problem solving, organization, and clear communication to ensure appropriate levels of care and timely documentation. This is a full-time position with a Monday–Friday schedule and local community impact.

Qualifications

• Minimum 22 years of age.
• 3 years combined experience and education.
• Bachelor’s degree with one year of full-time behavioral health experience.
• Valid AZ driver’s license and motor vehicle clearance.
• Fingerprint clearance or eligibility within eight weeks of hire.

Responsibilities

• Review documents and request required materials for appropriate level of care.
• Evaluate eligibility information for services and benefits.
• Develop service plans with consumers based on assessment and diagnosis.
• Maintain contact with utilization manager and out-of-home placement to ensure documentation and services.
• Coordinate care with the consumer’s care team to review interventions.
• Monitor mental status and update treatment plans.
• Provide crisis intervention and safety planning.
• Transition to appropriate level of care after treatment.
• Share information about community resources with consumers and families.
• Arrange transportation to services and appointments.
• Maintain thorough documentation of contacts and progress.
• Coordinate with PCP health plan, stakeholders, and funding sources.
