Job summary

Southwest Behavioral & Health Services in Payson is looking for a compassionate Care Coordinator to perform crisis stabilization and assist consumers in accessing resources. The ideal candidate will have a Bachelor’s degree and a year of experience in behavioral health, alongside excellent communication skills.

This position includes 3.3 weeks of PTO, 10 paid holidays, and various health benefits. Candidates must be at least 22 years old and hold a valid AZ driver's license.

Qualifications

• Minimum age of 22 years required.
• Combination of experience and education totaling 3 years is acceptable.
• Valid AZ driver’s license and motor vehicle clearance.
• Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to obtain within eight weeks of hire.

Responsibilities

• Perform crisis stabilization functions and assist consumers in accessing community resources.
• Complete necessary paperwork for referrals, care coordination, and out-of-home placements.
• Maintain contact with utilization managers for continued stay documentation.
• Provide face-to-face crisis intervention and monitors consumers' mental status.
