Job summary

Southwest Behavioral and Health Services is looking for a dedicated Care Coordinator in Prescott Valley to assist consumers in accessing community resources and perform crisis stabilization functions. This role entails monitoring client progress and ensuring compliance with treatment plans.

Qualifications

• At least 22 years old.
• Combination of experience and education totaling 3 years acceptable.
• Valid AZ driver's license & motor vehicle clearance required.
• Must have fingerprint clearance or be eligible within 8 weeks of hire.

Responsibilities

• Perform crisis stabilization functions for consumers.
• Assist in accessing community resources for clients.
• Monitor mental status of consumers and update treatment plans.
• Provide face-to-face crisis intervention and complete safety plans.
