Job summary

Family Health Center is seeking a Care Coordinator to connect behavioral health services, substance use treatment, primary care, and community resources. You will coordinate care across systems, assist with appointments, and address barriers such as transportation and housing in Pike, Martin, and Daviess counties.

The role supports ongoing engagement in care, with regular communication among care teams and community partners to improve health outcomes and stability over time.

Qualifications

• High school diploma or GED required.
• Valid driver’s license and insurability.
• 1–3 years in behavioral health or care coordination preferred.
• Experience serving underserved populations preferred.

Responsibilities

• Perform routine referral, linkage, coordination, and monitoring of care across service providers.
• Facilitate transitions through warm handoffs between levels of care.
• Monitor follow-up activities to track client engagement with treatment services.
• Coordinate with external providers such as PCPs, pharmacies, labs, and home health agencies.
• Ensure engagement with a primary care provider.
• Assist with accessing benefits and waivers.
• Support development and implementation of safety plans.
• Coordinate with crisis teams and emergency services when needed.
• Ensure timely follow-up after ER visits or hospitalizations.
• Document in the EHR.
