Job summary

Pillars Community Health is seeking a Care Coordinator to ensure continuity of client care across physical health, behavioral health, and social services. The role supports the HealthChoice Pathways to Success program for children and families, coordinating with care teams and community providers.

This position involves home/community visits, documentation in electronic health records, and collaboration with multidisciplinary professionals.

Qualifications

• Bilingual English/Spanish preferred.
• Excellent written and verbal communication skills.
• Experience with Outlook and Microsoft Office programs including Teams.

Responsibilities

• Coordinate care for Pathways to Success in wraparound or intensive care coordination models.
• Maintain documentation to meet organizational and service delivery requirements.
• Lead and participate in Child and Family Team meetings as required.
• Prepare and present care plans and related documentation.
