Job summary

LifeLong Medical Care in East Oakland is seeking a Community Health Worker to provide direct care services to a diverse patient population. The role reports to the Associate Behavioral Health Director or Center Director and is covered by SEIU-UHW under a collective bargaining agreement.

The position emphasizes referral coordination to therapy and psychiatry, coordination of behavioral health services, and support for patient engagement and portal access while maintaining patient privacy and data

Qualifications

• Bachelor’s degree in social work, health, or human services or equivalent experience.
• Administrative health or social service experience preferred.
• Knowledge of East Bay health and social resources.
• Experience with mental health or substance use services and homelessness is a plus.

Responsibilities

• Facilitate and manage referrals to therapy and psychiatry; connect patients to BH resources.
• Maintain provider schedules and optimize templates for productivity.
• Conduct warm hand-offs to integrate medical and behavioral health care.
• Support BH team operations and participation in meetings.
• Call reminders, track attendance, and reduce no-shows to aid continuity.
• Assist patients with screening forms and portal/video visits.
• Collect data and generate reports; support special projects.
