Job summary

Native American Health Center, Inc. is seeking a Care Coordinator I within Integration – Enhanced Care Management. You will collaborate with Behavioral Health teams to coordinate care across services, support members through navigation and warm handoffs, and promote holistic health for adults, youth, and families in Oakland.

The role emphasizes culturally sensitive planning, documentation, and collaboration with multiple care providers to improve health outcomes in a diverse community.

Qualifications

• Fluency in English and Spanish.
• Excellent writing and facilitation skills.
• Understanding of behavioral and primary health care integration.
• Knowledge of funding, community resources/services and clinical standards.
• Experience in care coordination, counseling, or case management.
• Valid California Driver’s License.

Responsibilities

• Manage a diverse caseload of adults, youth, and families meeting program criteria.
• Screen and assess members for mental health, substance use, and social needs.
• Develop, implement, and monitor individualized care plans using culturally sensitive approaches.
• Identify members needing treatment adjustment or case reviews.
• Complete documentation in electronic and case management systems.
• Meet mental health visit goals for contracts.
• Provide referrals and liaison services between agencies to support health needs.
