Job summary

LifeLong Medical Care’s East Oakland Health Center in Oakland, CA seeks a Population Health Specialist to support primary care teams with proactive panel management and patient follow-up, an entry-level role with learning opportunities.

The position is full-time, benefit eligible, 40 hours/week, and represented by SEIU-UHW. Salary is $22/hour with comprehensive benefits including medical, dental, vision, and paid holidays.

Qualifications

• Equivalent combination of experience, skills and/or education.
• 0-1 year experience with essential duties or duties similar to the above.
• Experience retrieving, organizing, and assessing quantitative data.
• Proficient in Microsoft Office Suite

Responsibilities

• Drive quality assurance and quality improvement of clinical quality measures with support.
• Maintain and share dashboards of key processes and outcome measures for QA/QI.
• Outreach to patients via phone and in person for chronic condition management or preventive care services.
• Oversee Social Determinants of Health (SDOH) improvement pilots and disseminate within the health center.
• Participate in meetings, trainings, and educational in-services; present data.
• Initiate case conferences and care plans with the care team to address challenges.
• Provide onboarding and ongoing training to other staff.
• Perform other job-related duties as assigned.
