Job summary

Honor Community Health is hiring a Full Community Health Worker to support Behavioral Health Home initiatives in Oakland County, MI. The CHW will work with patients and representatives to deliver six core services, ensuring care details are accurately recorded in the EHR and population health tools.

The role emphasizes effective communication, organization, and the ability to multi-task in a fast-paced setting while aiming to create an excellent patient experience and improve access to care.

Qualifications

• A High School Diploma or equivalent is required.
• Current MiCHWA Community Health Worker certification or the ability to obtain within 12 months of hire.
• Two (2) years of experience directly related to the duties and responsibilities specified or lived experience in the communities that Honor Community Health serves.
• Strong computer skills utilizing: Microsoft Office, EPIC, CHAMPS.

Responsibilities

• Deliver integrated, person-centered care through six core services: comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support, and referral to community and social support services.
• Track and follow up; document care details in the Electronic Health Record and population health management tool.
• Communicate effectively, stay organized, manage time efficiently, and multi-task to support patients and representatives.
