Job summary

Qualifications

• High school diploma or equivalent required.
• Community Health Worker certification preferred at hire; if not certified, certification must be completed after hire.
• Minimum of one year experience with individuals with disabilities or in related health/human services fields.

Responsibilities

• Assist individuals enrolled in Behavioral Health Home services to improve health and well-being.
• Build supportive relationships, encourage engagement in care plans, and coordinate care across departments.
• Navigate behavioral health and physical health systems and connect clients with resources and services.
• Advocate for clients and mentor them to take active roles in managing their health.
