Job summary

Qualifications

• AA or equivalent experience in a medical setting.
• Experience scheduling and documenting in EHR systems.
• Knowledge of medical terminology and standard software.
• Spanish and English language fluency.
• Ability to lift up to 20 pounds; physical requirements are listed.

Responsibilities

• Respond to in-clinic or outside referrals for behavioral health, psychiatric and substance abuse services as clinically indicated and based on urgency.
• Screen and triage patients for mental health and substance abuse disorders; facilitate follow-up care.
• Schedule and confirm appointments for behavioral health clinicians and group events; ensure full scheduling for maximum productivity.
• Facilitate referral linkage for services inside and outside the organization, including primary care, psychiatry, and related services.
• Provide patient education about mental health and treatment options; respond to warm handoffs in the clinic.
• Coordinate care across medical, dental, and behavioral health providers for referred patients.
• Systematically track referrals and patient progress using registries and reports; document encounters.
• Support behavioral interventions as needed, including evidence-based techniques.
• Document patient progress and treatment recommendations in the electronic record for communication with providers.
