Job summary

Qualifications

• Active ASW registration with the California Board of Behavioral Sciences.
• Strong oral, written, and electronic communication skills.
• Experience providing brief, evidence-based interventions in clinical or community settings.

Responsibilities

• Provide same-day warm handoffs and brief interventions within PCBH model.
• Collaborate with PCPs, medical assistants, and BH team; arrange referrals and communicate outcomes.
• Provide de-escalation, safety planning, and crisis intervention as trained.
• Document patient encounters consistently and maintain HIPAA compliance.
• Manage BH inbox/voicemail; schedule appointments and perform follow-ups.
