Job summary

Washoe Barton Medical Clinic in Gardnerville, NV is seeking a Behavioral Health Care Coordinator to join our outpatient behavioral health program. This full-time role focuses on comprehensive care management, assessment, care planning, and patient advocacy to improve engagement and outcomes.

The position emphasizes collaboration with a multidisciplinary team, care transitions, and community resource connections to reduce barriers and support recovery-oriented care for patients with complex needs.

Qualifications

• Bachelor’s degree in a mental health or related field required.
• Master’s degree in mental health field preferred.
• Community Health Worker preferred.
• Licensed Master Social Worker preferred.
• Non-Violent Crisis Intervention within 90 days of hire.
• Minimum two years of experience in behavioral health or related field.
• Strong de-escalation and communication skills.
• Knowledge of outpatient and inpatient behavioral health services.

Responsibilities

• Conduct comprehensive biopsychosocial assessments to identify patient strengths, needs, risks, and barriers to care.
• Develop, implement, monitor, and update individualized care management plans with patients and treatment team.
• Identify high-risk patients who may benefit from enhanced care management.
• Coordinate transitions of care between outpatient, inpatient, emergency department, primary and specialty care.
• Monitor patient progress and intervene when barriers to care are identified.
• Support patients in achieving improved health outcomes and quality of life.
• Utilize recovery-oriented approaches to support engagement and self-management.
• Maintain accurate documentation in the electronic health record.
