Molina Healthcare, Inc. in Florida is seeking a Behavioral Health Care Coordinator to support care management and coordinate member care across the continuum.

You will develop care plans with the member and care team, conduct visits, monitor progress, and promote integration of services to improve outcomes. The role requires a licensed clinician, strong communication, and travel up to 40% depending on state requirements. This is a full-time, on-site position with Molina's comprehensive benefits.

Qualifications

• 2+ years health care experience, preferably in behavioral health.
• Active and unrestricted health license per state requirements.
• Valid driver license and reliable transportation.
• Experience with severe mental illness and substance use disorders.
• Knowledge of whole-person care and discharge planning.
• Data entry with a clinical platform experience.
• Strong written and verbal communication skills.
• MS Office proficiency.

Responsabilités

• Completes comprehensive behavioral health assessments within regulated timelines.
• Develops and implements care plans with member, caregiver, and healthcare professionals.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents interventions and goal achievement.
• Maintains ongoing member caseload for outreach and management.
• Promotes integration of behavioral health, LTSS, and home/community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and Molina guidelines to motivate change.
• Assesses barriers to care and provides care coordination assistance.
• May provide consultation and recommendations to peers as needed.
• Travel 25-40% depending on state contracts.
