Job summary

You will collaborate with a multidisciplinary team to develop and implement individualized care plans aimed at improving member outcomes and cost-effective care. The role involves conducting assessments, home visits as needed, traveling up to 25–40% and using motivational interviewing to educate and motivate members.

Qualifications

• At least 2 years healthcare experience, preferably in behavioral health.
• Active, unrestricted clinical license as listed (LCSW, LMSW, APSW, CHES, LPC, LPCC, LMFT, PhD/PsyD, or equivalent).
• Valid driver’s license and reliable transportation for travel.
• Experience with severe mental health conditions and substance use disorders.
• Knowledge of whole person care, chronic conditions, and discharge planning.
• Data entry skills and experience with a clinical platform.
• Excellent verbal and written communication; proficiency in MS Office.

Responsibilities

• Provide care management and coordination with multidisciplinary team.
• Assess, plan, implement care plans with member and providers.
• Conduct telephonic, face-to-face or home visits as required.
• Monitor care plans and document interventions and outcomes.
• Maintain caseload and perform outreach.
• Promote integration of behavioral health, LTSS, and community resources.
• Facilitate care team meetings and ICT collaboration.
• Educate and motivate change using motivational interviewing and Molina guides.
• Identify barriers to care and address them.
• Travel up to 25-40% as required.
