Job summary

Red Rock Behavioral Health Services seeks a Care Manager to oversee implementation of comprehensive care plans for members with mental health, substance use, and physical health needs. The role involves crisis intervention, treatment planning, and ongoing care coordination within the Health Team.

Responsibilities include facilitating service plans, coordinating with providers, advocating for clients, and supporting families.

Qualifications

• BHCM I certification is required (or equivalent)
• High School Diploma required; bilingual preferred
• Valid Oklahoma Driver License and clean MVR
• Experience with community services and health agencies preferred

Responsibilities

• Provide advocacy, coordination, and support to help consumers obtain daily living necessities
• Attend disposition/treatment team meetings and contribute relevant information
• Develop, implement, and revise Service Plan goals and objectives
• Coordinate services and communicate with DHS and referral sources
• Educate and support consumers' families and assist with primary care coordination
• Document progress and maintain client records; ensure compliance with Medicaid and ODMHSAS
