Job summary

Integrated Services for Behavioral Health (ISBH) in Ohio seeks an Assessment Specialist to deliver comprehensive behavioral health screenings, assessments, and diagnoses across the lifespan, linking clients to community-based and telehealth services and supporting person-centered care.

Requirements: high school diploma, bachelor’s degree in social work or related field, and Ohio licensure (LSW/LPC) with LISW/LPCC/PhD preferred; CANS certification required.

Qualifications

• Experience with multi-need individuals and families.
• Broad knowledge of community service systems.

Responsibilities

• Screens, assess, and diagnose across the lifespan to inform service recommendations.
• Link clients to community-based and telehealth services.
• Maintain knowledge of ISBH programs, eligibility criteria, and workflows.
• Use data and analytics to monitor referrals and caseload flow.
• Complete documentation and billing requirements per standards.
• Collaborate with internal and external partners for coordinated care.
