Job summary

OhioHealth is seeking a Behavioral Health Therapist to support and enhance biopsychosocial functioning through assessments, treatment planning, and therapy across individual, family, and group modalities. The role requires crisis intervention, discharge planning, and strong interdisciplinary collaboration with physicians, case managers, nurses, and other care team members.

Minimum qualifications include a Master’s in Social Work and licensure/certification as LISW, LPCC, or related credentials;

Qualifications

• Master's degree in Social Work required.
• Ability to identify appropriate community resources on assigned caseload.
• Interdisciplinary collaboration with patients, families and team to achieve outcomes.

Responsibilities

• 30% Advanced Clinical Assessment Conducts patient/family/significant other interviews, chart reviews, and professional collaboration to obtain and interpret information pertaining to patient/client biopsychosocial functioning.
• 30% Advanced Intervention Management Develops a plan of intervention based upon assessment to resolve identified problems and/or to support and enhance biopsychosocial functioning.
• 25% Advanced Population Management Coordinate clinical and financial outcomes of an assigned population with interdisciplinary staff and external organizations.
• 10% Documentation and Regulatory Compliance Document assessments and interventions in patient records per department standards and regulatory requirements.
• 5% Maintains awareness of current hospital and community services to facilitate linkages for patient care.
