Job summary

Qualifications

• Master's Degree in Social Work required.
• Licenses: DL-HC, LISW, LPCC as applicable depending on department.
• Must demonstrate knowledge of biopsychosocial assessment and treatment planning.

Responsibilities

• Conduct biopsychosocial assessments and formulate treatment plans.
• Provide individual, family, or group therapy and discharge planning.
• Coordinate care with interdisciplinary team and community resources.
• Maintain accurate documentation per regulatory standards.
