Qualifications

• High school diploma or GED required.
• Bachelor's degree in counseling, social work, or related field required.
• Master's degree in counseling, social work, or related field preferred.
• Active Ohio license as LSW or LPC required.

Responsibilities

• Conduct biopsychosocial assessments and provisional diagnoses under supervision.
• Engage individuals and families in home/community to identify strengths and goals.
• Provide therapeutic interventions within licensure and supervision.
• Develop and update treatment plans using trauma-informed strengths-based principles.
• Coordinate with internal and external teams to ensure continuity of care.
• Maintain timely, accurate, compliant documentation.
• Travel within designated service area and participate in required training.
