Job summary

LifePath Systems seeks an Assessment Specialist to serve as a specialized case manager for individuals and families with behavioral health needs, guiding TRR assessments, PCRP, and enrollment documentation in clinical settings or community-based environments.

Collaborating with Youth and Family Services and Adult Services, you will ensure timely completion of assessments, consignments, and record updates while maintaining high professional standards and accurate EHR documentation.

Qualifications

• Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
• At least two years of related mental health experience and/or training.
• Must have reliable transportation and a valid driver’s license.

Responsibilities

• Provide person-directed case management to assist in navigating medical, social, vocational, financial, educational, and other individualized services.
• Initiate and collaboratively complete TRR and PCRP assessments and consent documentation prior to expiration as needed.
• Monitor upcoming expiration dates and ensure timely completion of service notes and documentation in the EHR.
• De-escalate behavioral health crisis situations in a calm and respectful manner.
