The Assessment Specialist serves as a specialized case manager for individuals and their families with behavioral health needs in order tohelp guide treatment preferences and recovery-oriented services. This position will be responsible for overseeing the timely completion ofthe Texas Resilience and Recovery (TRR) assessments, Person Centered Recovery Plans (PCRP), annual consents, and various otherdocumentation and assessment tools needed for enrollment in behavioral health services. This position will require ability to work outsideof normal business hours in office or community-based settings in order to meet individual and family needs and preferences. This positionwill monitor upcoming expiration dates for those seeing clinic prescribers, med distribution and pickup, as well as support intake forcompletion of initial recovery plan development. The Assessment Specialist will work collaboratively with program leadership in both Youthand Family Services and Adult Services to meet the unique needs of those served.This Assessment Specialist exemplifies service excellence and will be available as needed to support the case management team meetthe needs of individuals in services through seeing individuals in crisis, taking walk-in appointments, filling in for case managers, orupdates needed prior to prescriber appointments. Is responsible for initiating and collaboratively completing TRR and PCRP prior toexpiration and as needed based on individual change. Engagement with individuals served and exceptional customer service will beprovided and documented in the electronic health record.This Assessment Specialist practices good stewardship of resources by ensuring all required trainings are completed prior to due date andcenter-issued equipment is kept in good repair. Provides person directed case management to assist in navigating access to medical,social, vocational, financial, educational, and other individualized services. Integrity is necessary to ensure adherence to the Health andHuman Services Commission, Texas Administrative Code, Medicaid billing rules, contracts, grants, and Lifepath Systems policies andprocedures.This Assessment Specialist builds positive relationships with community-facing agencies and always maintains a professional relationshipwith all community stakeholders and partners. Professional conduct with Peers, Leadership and Support teams is expected at all times.Promoting choice and autonomy through a strength-based model is necessary to assist individuals served with improving access to socialdeterminants of health.Working independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served andcommunity partners; to facilitate improvement in measurable ways, is central to LifePath Systems mission to serve. Ability to assess andde-escalate behavioral health crisis situations in calm and respectful manner.Specific Targets: At minimum, complete 2.5 TRR/PCRP and/or crisis assessments/bh screening a day and 200 contact attemptsper month. Expired TRR/PCRP and auto discharges will be monitored for decrease month-over-month. 90% of service notes aresigned within 2 business days of service. All audit scores exceed 80% quality standard. All required trainings are completed prior todue date.If Bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approvedlanguage in the provision of services as needed.Education, Training and Experience (including licensure and certification)Bachelor’s degree in psychology, social work, or related field from a four-year college or university.At least two years of related experience and/or training in the field of mental health preferred.Must have reliable transportation and a valid driver’s license.Knowledge, Skills, and AbilitiesKnowledge and experience in providing case management, skills training, and symptom management services to individuals with mental illnessAbility to assess individual needs, to coordinate services, and to communicate effectively.Knowledge of the social services system and the ability to connect individuals with appropriate services.Ability to write clearly and professionally.Ability to think clearly and logically to deal with potentially dangerous and/or emotional situations
