Job summary

LifePath Systems seeks an Assessment Specialist to serve as a specialized case manager for individuals and families with behavioral health needs. You will guide treatment preferences, complete TRR assessments, PCRP plans, annual consents, and related documentation to enroll clients in services.

You will work with Youth and Family Services and Adult Services, providing crisis support, conducting walk‑in appointments, and ensuring timely notes and proper resource referrals while maintaining

Qualifications

• Bachelor's degree in psychology, social work, or related field.
• At least two years of related experience and/or training in the mental health field preferred.
• Must have reliable transportation and a valid driver’s license.

Responsibilities

• Oversee timely completion of TRR assessments and PCRP and annual consents.
• Support enrollment in behavioral health services and document in the health record.
• Collaborate with Youth/Family and Adult Services leadership to meet service needs.
• Provide crisis assessment, walk‑in coverage, and case management support as needed.
• Ensure trainings are completed by due dates and resources are properly maintained.
