Job summary

Integratedservice is seeking a qualified professional for a supervisory referral/health coordination role in Ohio. The position emphasizes leadership, policy oversight, and ensuring compliant, accurate client documentation in an EHR environment.

The role requires a Master's or higher in social work or clinical counseling and active licensure (LISW or LPCC) in Ohio, with at least 3 years in healthcare or related fields and supervisory experience preferred.

Qualifications

• High school diploma or GED required.
• Bachelor's degree in social work or clinical counseling required.
• Master's degree in social work or clinical counseling required.
• Active LISW or LPCC in Ohio required.

Responsibilities

• Monitors timeliness, accuracy and follow-up of referral specialist.
• Ensures quality and timeliness in written and verbal communication with clients and community partners.
• Responsible for performance management, recruitment, onboarding, and team development, including progressive discipline when necessary.
• Provides routine support and assistance to employees through conferences, analysis of problems, and improving helping skills.
• Leads agency-wide collaboration and innovation to support desired outcomes.
• Implement and oversee organizational policies, procedures, and processes.
• Participates in outreach activities such as follow-up or reminder calls as necessary.
• Maintain compliance with HIPAA and mandated reporting requirements.
• Utilizes databases and EHR systems for accurate client documentation.
• Travel within designated service areas as needed.
• All other duties as assigned.
