Job summary

Open Sky Community Services is seeking a Clinical Access & Triage Coordinator/ Behavioral Health Triage Coordinator to help individuals access treatment and community resources. You will collaborate with medical and behavioral health teams to implement support plans and facilitate rapid access to care for high-need cases.

Qualifications

• High School Diploma, GED or equivalent is required.
• Valid Driver’s License with acceptable driving record required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Assist individuals with intake packet completion, enrollment requirements, and other admission-related paperwork.
• Conduct comprehensive assessments, develop individualized support plans, monitor progress, and document in accordance with program and regulatory requirements.
• Exercise sound clinical judgment by identifying and responding to risk-related situations and seeking consultation when necessary.
• Conduct outreach to community providers to establish and strengthen partnerships, expand referral networks, and improve access to services.
• Review referrals received via phone, fax, email, and triage platforms, and determine service eligibility in collaboration with sources.
• Coordinate and facilitate referrals to appropriate community-based resources and support services based on identified needs.
• Develop and maintain a comprehensive network of community resources to support members' treatment, recovery, and social service needs.
• Serve as a liaison among members, families, community agencies, and healthcare providers to coordinate care and ensure access to needed services.
• Support Rapid Response Hub referrals and related care coordination activities.
• Triage incoming calls, assess needs, and provide guidance, support, and appropriate referrals.
• Assist individuals with scheduling and attending initial PT-1 transportation appointments and other access-to-care needs.
• Support data collection, documentation, and reporting for SAMHSA grants, including SUPRT Tools and related activities.
• Monitor engagement in services, identify barriers to care, and collaborate with partners to support outcomes.
