Qualifications

• Board certified with Maryland NP or PA certification.
• 1–2 years of primary care experience, geriatric patients optional.
• Strong communication and independent problem-solving skills.

Responsibilities

• Provide high quality primary and palliative care to home-based patients in and around Baltimore.
• Perform physical examinations and develop care plans.
• Coordinate with office staff and clinical operations across platforms.
• Prescribe medications and necessary interventions.
• Order diagnostic tests as needed.
