Job summary

PurposeCare is seeking an experienced Home Health Registered Nurse (RN) for a full-time role in the Midwest's home health network. You will manage a patient-focused caseload, deliver high-quality nursing care in patients' homes, and collaborate with physicians, therapists, and a multidisciplinary team.

Your responsibilities include comprehensive assessments, care planning, patient education, and OASIS documentation, with emphasis on autonomy and clinical excellence in a home setting.

Qualifications

• Active RN license and current licensure to practice in Illinois.
• Minimum of one year of Home Health experience.
• Solid knowledge of OASIS documentation and Medicare regulations.
• Excellent assessment and clinical decision-making skills.
• Outstanding communication and relationship-building abilities.
• Valid driver’s license, reliable transportation, and auto insurance.
• Passion for delivering patient-centered care in the home setting.

Responsibilities

• Complete Start of Care, Resumption of Care, and Recertification assessments.
• Develop individualized care plans to support patient goals and independence.
• Coordinate care with physicians, therapists, social workers, and caregivers.
• Educate patients and families on disease management, medications, and safety.
• Complete timely documentation and OASIS assessments.
• Monitor progress and adjust care plans as needed.
• Supervise Home Health Aides and collaborate with LPNs.
• Participate in case conferences and interdisciplinary care planning.
• Help improve patient outcomes while complying with Medicare Conditions of Participation.
