Job summary

VitalCaring Group is seeking a Registered Nurse (RN) for Home Health, serving patients in their homes with strong clinical autonomy. You will lead care plans, coordinate with physicians and therapy teams, and drive outcomes from admission to discharge.

Ideal candidates have an active RN license, CPR, and at least 1 year of RN experience in home health or related settings. The role requires excellent communication, time management, and the ability to manage caseloads independently.

Qualifications

Active RN license (or compact eligibility) and CPR certification.
Graduate of an accredited nursing program.
Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
Strong clinical judgment and ability to think independently.
Excellent communication skills and patient-centered approach.
Ability to manage a caseload and prioritize effectively.
Proficiency with EMR systems.
Valid driver's license and reliable transportation.

Responsibilities

Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
Develop and manage individualized care plans in collaboration with physicians and care teams.
Deliver skilled nursing care, including medication management, treatments, and interventions.
Serve as the case manager, coordinating care across therapists, aides, and physicians.
Monitor patient progress and make clinical decisions based on changing conditions.
Educate patients and families to support disease management and independence.
Coordinate care transitions and discharge planning to reduce readmissions.
Maintain timely, accurate documentation in the EMR.
Supervise and support Home Health Aides as needed.
Participate in on-call rotation as appropriate.
