Job summary

PurposeCare is seeking an experienced Home Health Registered Nurse (RN) to deliver patient-focused care in home settings across the Midwest. You’ll manage a patient caseload with autonomy, coordinating with physicians, therapists, and the interdisciplinary team to ensure safe, effective, and compassionate care.

The ideal candidate has an active RN license, at least one year of Home Health experience, and a solid understanding of OASIS and Medicare requirements.

Qualifications

• Active Registered Nurse (RN) license required.
• Minimum of one year of recent Home Health experience.
• Strong knowledge of OASIS documentation and Medicare regulations.
• Excellent assessment and clinical decision-making skills.
• Outstanding communication and relationship-building abilities.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Complete Start of Care, Resumption of Care, and Recertification assessments.
• Develop individualized care plans that support patient goals and independence.
• Coordinate care with physicians, therapists, social workers, and caregivers.
• Educate patients and families on disease management, medications, and safety.
• Complete timely documentation and OASIS assessments.
• Monitor patient progress and adjust care plans as clinical needs change.
• Supervise Home Health Aides and collaborate with LPNs as appropriate.
• Participate in case conferences and interdisciplinary care planning.
• Help improve patient outcomes while maintaining Medicare compliance.
