Qualifications
- Current license with the state Board of Nursing.
- 1 year of nursing experience is required - home health and OASIS experience preferred.
- Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing.
- Current Driver's License, vehicle insurance, and/or access to dependable transportation.
Responsibilities
- Provide skilled home health visits to patients in their homes.
- Assess patient needs and develop care plans in collaboration with families.
- Coordinate with caregivers and other healthcare professionals to ensure seamless care.
- Maintain accurate and timely documentation of visits and patient progress.
