PurposeCare is a leading Midwest home health provider seeking an experienced Registered Nurse (RN) for full-time Home Health. You will oversee patient care in-home, collaborate with physicians and therapists, and develop individualized plans while maintaining Medicare standards.

Qualifications

• Active RN license in Illinois (or eligible).
• Minimum 1 year of Home Health experience.
• Strong knowledge of OASIS documentation and Medicare regulations.
• Excellent assessment and clinical decision-making skills.
• Outstanding communication and relationship-building abilities.

Responsibilities

• Oversee a caseload of patients within assigned territory.
• Develop individualized care plans aligned with patient goals.
• Coordinate care with physicians, therapists, social workers, and caregivers.
• Educate patients and families on disease management and safety.
• Complete Start of Care, Resumption of Care, and Recertification assessments.
