The role requires active nursing licensure, at least one year of nursing experience (home health preferred), and the ability to perform physically demanding tasks.

Qualifications

• Current license with the state Board of Nursing.
• 1 year of nursing experience is required - home health and OASIS experience preferred.
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing.
• Current Driver's License, vehicle insurance, and/or access to dependable transportation.
• Ability to successfully complete a background check.

Responsibilities

• Provide home health nursing visits per plan of care.
• Assess patient needs and adjust care with physician oversight.
• Document care and outcomes.
• Maintain patient safety and infection control.
