Qualifications
- Active RN license in Illinois (or eligible).
- Minimum 1 year of Home Health experience.
- Strong knowledge of OASIS documentation and Medicare regulations.
- Excellent assessment and clinical decision-making skills.
- Outstanding communication and relationship-building abilities.

As a Home Health RN, you'll oversee a caseload of patients within your assigned territory while partnering with physicians, therapists, and interdisciplinary team members to deliver exceptional care.
Responsibilities include:
- Complete comprehensive Start of Care, Resumption of Care, and Recertification assessments.
- Develop individualized care plans that support patient goals and independence.
- Coordinate care with physicians, therapists, social workers, and caregivers.
- Educate patients and families on disease management, medications, and safety.
- Complete timely documentation and OASIS assessments.
- Monitor patient progress and adjust care plans as clinical needs change.
- Supervise Home Health Aides and collaborate with LPNs as appropriate.
- Participate in case conferences and interdisciplinary care planning.
- Help improve patient outcomes while maintaining compliance with Medicare Conditions of Participation and agency quality standards.

The ideal candidate has:
- Active Registered Nurse (RN) license
- Minimum of one year of recent Home Health experience
- Strong knowledge of OASIS documentation and Medicare regulations
- Excellent assessment and clinical decision-making skills
- Outstanding communication and relationship-building abilities
- Valid driver's license, reliable transportation, and auto insurance
- Passion for delivering patient-centered care in the home setting
