Qualifications

• Graduate of an accredited Physical Therapy program.
• Current PT license in the state of practice.
• Minimum 1-2 years of experience, preferably in home health.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status and rehabilitation potential.
• Develop individualized care plans in collaboration with the care team.
• Monitor patient progress and adjust treatment plans as needed.
• Supervise Physical Therapist Assistants (PTAs) as per state guidelines.
