Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in home settings, evaluating functional status, designing plans, and guiding patients toward independence.

You'll collaborate with physicians, nurses, and PTAs, supervise PTAs per state guidelines, educate families, and maintain accurate documentation in the EMR. A valid PT license in Texas and CPR are required.

Qualifications

• Graduation from an accredited Physical Therapy program (DPT or Master’s).
• Current Texas PT license.
• Minimum 1–2 years of home health experience preferred.
• CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Document patient care and progress accurately and timely within the EMR.
• Partner closely with nurses, therapists, and physicians to ensure coordinated care.
