Job summary

VitalCaring is seeking a Home Health Physical Therapist (PRN) to deliver patient-centered care in home settings. You will evaluate, treat, and educate patients to regain mobility and independence, while coordinating with nurses, physicians, and the care team.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking.
• Excellent communication and patient education abilities.
• Current CPR certification.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance and independence.
• Educate patients and caregivers on treatment plans, exercises and safety strategies.
• Monitor progress and adjust plans; support safe transitions of care.
• Document care and progress accurately in the EMR; participate in team meetings.
• Supervise PTAs per state guidelines; mentor caregivers and support staff.
