Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients’ homes across Texas. You will evaluate mobility, develop plans with physicians, and provide hands-on therapy to improve independence.

Ideal candidates have 1–2 years of home health experience, current PT license, CPR certification, and strong communication skills. The role offers autonomy, collaboration with nurses and physicians, and opportunities for growth.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• 1–2 years of home health experience preferred
• Current CPR certification

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
