Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to evaluate patients, develop personalized care plans, and provide hands-on therapy at patients’ homes in Texas. You will collaborate with physicians and the care team to support independence and safe mobility.

Candidates should have a DPT or Master’s in Physical Therapy, hold a current Texas PT license, and have 1–2 years of home health experience preferred. CPR, driver’s license, and reliable transportation are required.

Qualifications

• Proven ability to assess patient function, mobility, and rehab potential.
• Strong critical thinking to adjust plans and optimize outcomes.
• Excellent communication with patients, families, and care teams.
• Ability to educate patients on plans, exercises, and safety protocols.
• CPR certification and valid license to practice in state of Texas.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Supervise PTAs in accordance with state guidelines.
• Document patient care and progress in EMR.
• Participate in case conferences and team meetings.
