21st Century Home Health Services is seeking a Speech Therapist/Language Pathologist to deliver in-home care in Auburn and Colfax, CA. You9ll assess and treat communication, swallowing and cognitive disorders as part of a collaborative interdisciplinary team, guided by case management and patient-centered goals.

Qualifications

• Active CA Speech Therapist license and unrestricted practice.
• BLS/CPR certification required.
• Valid CA drivers license and clean driving record.

Responsibilities

• Provide skilled speech, language, cognitive and swallowing services in patients' homes.
• Evaluate and treat disorders affecting communication as part of the interdisciplinary care team.
• Collaborate with Case Manager and educate patients and families.
• Participate in ongoing quality and regulatory requirements.
