Job summary

Iconic Care Support Services in Ogden, UT seeks a Physiatry (PM&R) NP/PA to relocate to Redding, CA and provide onsite care at contracted facilities. You will round 1–3 SNFs, treating 25+ patients daily with injections, Botox, and meds, operating autonomously with a collaborative team.

Qualifications

• Current, unrestricted PA or NP license.
• DEA licensed or willing to obtain DEA licensure.
• Nursing home and/or physiatry experience is a plus, but not required.
• Comfort with peripheral joint injections (knees, shoulders, hips).
• Strong communication skills and ability to collaborate with teams.
• Familiarity with EHR charting.

Responsibilities

• Rounding at 1–3 Skilled Nursing Facilities or rehab centers, 1–3 days per week at each location.
• Deliver physiatry care to 25+ patients per day.
• Provide interventions such as joint injections, Botox, dry needling, and medication management.
