Job summary

21st Century Home Health Services is seeking an Occupational Therapist to provide skilled in-home evaluations and therapy in Monterey County. You will work with a Case Manager to address home safety, wheelchair seating, pain management, falls prevention, and chronic disease management, helping clients live independently.

The role requires an active CA OT license, BLS/CPR, and a CA driver's license. You will join a team-focused organization with strong patient outcomes and flexible scheduling.

Qualifications

• Current unrestricted CA Occupational Therapist License is required.
• BLS/CPR certification preferred.
• Valid CA Driver's License required.
• Experience working with an interdisciplinary care team is highly desired.

Responsibilities

• Provide skilled OT services in patients' homes in partnership with the Case Manager.
• Evaluate home safety, mobility, activities of daily living, and caregiver training.
• Participate in care planning to support safe, productive home environments.
