Job summary

Longevity Health in Greenville, SC is seeking a Nurse Practitioner or Physician Assistant with a passion for geriatrics in long-term care. You’ll manage a panel of enrolled members, conduct comprehensive assessments, and develop individualized care plans for each resident.

You’ll participate in nursing facility care team meetings, maintain frequent communication with families, and ensure the care team remains aligned with members’ goals.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• MSN as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program.
• Experience in nursing home environments is strongly desired.
• Computer/Technology proficiency required.
• Ability to engage and communicate effectively with physicians, members, and family members.
• Self-directed but functions well in a team environment.
• Excellent clinical skills—assessment, diagnosis, treatment, prescribing for chronically ill population.
• Demonstrated critical thinking, organization, and prioritization skills.

Responsibilities

• Provides collaborative primary care and services to a panel of members.
• Delivers ISNP Model of Care activities and develops advanced care planning to avoid unnecessary hospitalizations.
• Manages health plan operations: prior authorization, Part A/B services, medication reviews, quality gap closure, and promotes membership growth.
• Serves as a trusted advisor to members, families, and nursing home staff and colleagues.
• Communicates with members’ PCPs, families, and care teams about goals of care and treatment plan changes.
• Maintains member-centered care and supports overall market goals while balancing caseloads.
• Represents Longevity Health to stakeholders in nursing facilities and the community.
• Functions autonomously and is accountable for practice in the nursing home environment.
• Establishes meaningful relationships and effective collaboration within the facility.
