Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to oversee a panel of long-term care members. You will lead assessments, develop individualized care plans, and coordinate with the care team to align with each member’s goals.

You will collaborate with clinicians and leadership to deliver high-quality care, engage families, and support Medicare/ISNP operations while practicing autonomously in a nursing home setting.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• Master of Science in Nursing as a Nurse Practitioner or Master’s Degree from a nationally accredited program.
• Experience working in the nursing home environment is strongly desired.
• Computer/Technology proficiency required.
• Ability to engage and communicate effectively with physicians, members, and family members.
• Self-directed but works well in a team environment.
• Excellent clinical skills—assessment, diagnosis, treatment, and prescribing for chronically ill patients.
• Strong critical thinking, organizational, and prioritization abilities.

Responsibilities

• Provides collaborative primary care to a panel of members.
• Delivers ISNP Model of Care activities and advanced care planning.
• Manages authorization, Part A/B services, medication reviews, and quality gap closure.
• Acts as a trusted advisor to members, families, and staff.
• Maintains clear communication with PCPs, families, and care teams.
• Supports market goals and balances member caseloads.
• Represents Longevity Health to facilities and stakeholders.
