Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to care for seniors in long-term care facilities. You will manage a patient panel, perform comprehensive assessments, and develop individualized care plans with a strong emphasis on communication with families and the care team.

Qualifications

• MSN in Nursing as a Nurse Practitioner or related master’s degree.
• State license as an Advanced Practice Nurse or PA with NP/PA board certification.
• Experience in nursing home environments is strongly desired.
• DEA license and active NPI/Medicare provider numbers required.

Responsibilities

• Provide collaborative primary care to a panel of long-term care members.
• Conduct comprehensive assessments and create individualized care plans.
• Engage in care team meetings and coordinate with families and PCPs.
• Support HEDIS/STARs quality initiatives and manage care plans.
• Maintain autonomous practice while aligning with team goals.
