Job summary

UnitedHealth Group's Optum Senior Community Care team seeks an NP or PA to provide longitudinal care to medically complex patients in community-based settings across Marion County, Oregon. You will be the clinical decision-maker with a supportive interdisciplinary team, emphasizing quality and holistic care rather than volume.

The role is per diem and field-based, offering autonomy, a flexible schedule (minimum 24 hours/month), and no productivity quotas.

Qualifications

• Active ANCC or AANP certification for NPs or NCCPA certification for PAs
• Unrestricted licensure in practicing state and driver’s license
• Access to reliable transportation

Responsibilities

• Conduct comprehensive patient assessments
• Manage acute and chronic conditions with tests, diagnostics and interventions
• Develop and manage complex treatment plans
• Lead goals-of-care discussions and advance care planning
• Collaborate with interdisciplinary care team and caregivers
• Provide patient and caregiver education
• Ensure accurate clinical documentation
