Job summary

UnitedHealth Group’s Optum Home & Community Care seeks NP/PA clinicians to provide per diem, field-based longitudinal care in skilled nursing facilities across Lane County, Oregon. You will make clinical decisions with strong interdisciplinary support, while avoiding productivity quotas and maintaining work-life balance.

You’ll build long-term relationships with patients and families, manage complex conditions, and lead care planning with PCP collaboration in a value-based model.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PAs: NCCPA certification and state licensure
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations
• Active and unrestricted driver’s license
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
• Ability to gain a collaborative practice agreement, if applicable in your state
• Ability to navigate varied environments and to position oneself as needed to perform job duties

Responsibilities

• Conduct comprehensive patient assessments
• Manage acute and chronic conditions, including testing, diagnostics and interventions
• Manage members during a short term sub-acute rehabilitation episode
• Develop and manage complex treatment plans
• Lead advanced illness, goals-of-care and advance care planning conversations
• Collaborate with the interdisciplinary care team, facilities and caregivers
• Provide patient and caregiver education
• Ensure accurate and complete clinical documentation
