Job summary

Optum is seeking an NP or PA to join our Senior Community Care team in Marion County, OR. You will provide patient care in home and community settings, conduct comprehensive assessments, and collaborate with an interdisciplinary team to build and execute care plans.

The role emphasizes quality outcomes over volume, with a manageable 4–6 patient visits per day and a flexible schedule that supports work-life balance and no on-call requirements.

Qualifications

• Graduate of an accredited MS in Nursing or Doctor of Nursing Practice program.
• Active and unrestricted NP license in the state of practice or ability to obtain by start date.
• Board certification in a recognized NP specialty or NCCPA certification for PAs.
• Active DEA license or ability to obtain prescriptive authority per state regulations.

Responsibilities

• Conduct comprehensive assessments of patients in home/community settings.
• Manage patients through acute events or chronic condition exacerbations.
• Ensure accurate documentation including CMS and ICD-10 coding.
• Collaborate with interdisciplinary care team to coordinate care.
• Lead advanced illness and end-of-life discussions when appropriate.
• Provide counseling and education to patients and caregivers.
• Document findings concisely and maintain compliant care plans.
• Coordinate care with facility staff and families as patients transition settings.
• Review orders for appropriateness and adjust plans per patient needs.
