UnitedHealth Group is seeking an Advanced Practice Clinician (NP/PA) to provide longitudinal care across community settings in Multnomah and Clackamas Counties. You will make clinical decisions with strong interdisciplinary support and manage your own patient panel and schedule.

The role emphasizes autonomy in patient care, with no on-call, weekends, or holidays required, and a flexible schedule. Join a high-quality, value-based model focused on outcomes and work-life balance.

Competenze

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• Active and unrestricted NP/PA license in state of residence or ability to obtain by start date.
• Board certification by relevant nursing board or NCCPA for PA role and DEA licensure.
• Ability to obtain collaborative practice agreement where required.

• Conduct comprehensive patient assessments
• Manage acute and chronic conditions with diagnostics and interventions
• Develop and manage complex treatment plans
• Lead advanced illness, goals-of-care and advance care planning conversations
• Collaborate with interdisciplinary care team, facilities and caregivers

• Ensure accurate and complete clinical documentation
