Job summary

Northeast Healthcare Recruitment, Inc. seeks an experienced Nurse Practitioner or Physician Assistant to join a Skilled Nursing Facility clinical team on a full-time basis. You will manage your own panel, see 16–20 patients daily, and coordinate with physicians and facility staff to ensure seamless care.

Work 6-hour shifts with morning rounds, no on-call or overnights, and the ability to chart from home. Productivity-based compensation offers high earning potential; additional long-term

Qualifications

• Board certification required (e.g., AGNP, FNP for Nurse Practitioners; PA-C for Physician Assistants) — active and unrestricted.
• Valid, unrestricted Nurse Practitioner or Physician Assistant license in the state of operation.
• Master's degree in Nursing (Nurse Practitioner) or from an accredited Physician Assistant program — required for the corresponding credential.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–20 patients a day, including long-term chronic care and acute issues.
• Manage annual wellness visits and H&Ps, review meds and orders, and coordinate with collaborating physicians, specialists, and facility staff.
• Stay in touch with families and join care conferences where it makes sense.
