Job summary

Abode Care Partners in Henrico, VA is seeking a compassionate Nurse Practitioner to join our home-based primary care team. You will conduct clinical assessments, develop personalized care plans, and manage chronic conditions in seniors and complex patients within home settings.

Qualifications

• Active state license as NP/PA or PA license if applicable
• MSN is required for Nurse Practitioners; PA does not require it
• Current CPR certification
• Active DEA license
• Strong communication and time management skills
• Proficiency with EMR systems and Microsoft Office

Responsibilities

• Provides evidence-based medical care including planning and evaluation of patient needs
• Treats chronic and acute conditions and documents accordingly
• Orders and interprets lab and diagnostic tests
• Collaborates with interdisciplinary team for a comprehensive care plan
• Provides referrals when necessary and ensures up-to-date medical records
